LMS Physical Information
Please note that all athletes must have a physical completed and dated after April 1 of this year in order to participate in any camp, practice or tryout. The physical form must be given to the respective coaches if not already on file with the Liberty Middle School Athletic Dept. The physical form for students entering grades 7-8 and those students transitioning to 9th grade may be found below or on the CHS Athletics website under more. LMS uses the same IHSAA Physical Form as CHS.
*Pop Warner physicals are not acceptable.

Physicals / Registration Information
- Most forms are signed when Parents and Students register with Final Forms.

The Consent and Release Certificate signed by both the Parent and student must be turned in along with the Physical Examination completed by the Doctor. This is the only paperwork that needs to be turned in to the coach or AD.

Please save your Final Forms password for future use. This procedure is used from 6th to 12th grade.
